Thank you for your interest in becoming a Certified Play Therapist or in becoming a Certified Play Therapist Supervisor.
Below, please find the forms you will need to complete this process, as well as a guide to assist you in completing the forms. All forms are fillable pdf’s.

It is very important that you read the "How to Complete Guide", as it is a useful document to assist you in completing your application and answering the many questions you may have.
